- Change of Graduation Date (used when a student who has already applied for graduation needs to change the anticipated graduation date to a different semester).
- Degree Verification (a form to request a letter indicating all degree requirements have been satisfied before the degree is posted to a student’s official transcript).
- Enrollment Verification (a form to request a letter indicating that you are/were an enrolled student).
- FERPA Directory Information (withholds or releases disclosure of Directory information).
- FERPA Release Authorization (releases student Education Records to a designated third party).
- Graduate Academic Progress Plan (used when a program desires to retain a student who is not meeting University standards and is at risk for dismissal, or to reinstate a student who was just dismissed back into the program.)

- Leave of Absence Request (to request temporary suspension of usual inactivation practices, for up to six consecutive semesters).
- Preferred First Name (Used to add a preferred first name to a student's record)
- Program Change Request (a form to request a program change from one graduate program to another).

- Student Billing/Late Withdrawal Petition (Used to petition for a bill adjustment or late withdrawal due to extenuating circumstances.)
