International students must meet certain vaccination requirements for a variety of diseases, including COVID-19. Before they arrive, all international students must fill out this vaccination form and send it, along with evidence of each vaccination (including Covid-19 vaccination), to NEIU Health Services at health-services@neiu.edu.
Any questions about what kinds of proof of vaccination are accepted at NEIU, as well as any other specifics regarding vaccines and medical issues, can be directed to Student Health Services.
